This form is to report a warranty repair - please refer to you warranty documents for items covered.

    Declaration
    1. If all information required by the Warranty Claim Form is not provided in full, Appley will not be able to begin the claim process which could delay your claim and/or result in it being rejected.
    2. Appley may take up to five working days to assess your query.
    3. The warranty will only be honoured if there are no outstanding invoices on account.
    4. Any site visits arranged for items not covered under the Warranty, or as a result of negligence or misuse, will be chargeable, please see our rates below.
      • upto 75 miles from Head Office - £250 + VAT
      • upto 150 miles from Head Office - £400 + VAT
      • 150+ miles from Head Office - £600 + VAT

    Site Visit Charges

    Any site visits arranged for items not covered under the Warranty, or, are as a result of negligence or misuse will be chargeable, please see our rates below.

    • upto 75 miles from Head Office - £250 + VAT
    • upto 150 miles from Head Office - £400 + VAT
    • 150+ miles from Head Office - £600 + VAT
